Is this a real card that has been altered by Zodiac or was this whole card made by him?

I have never really put much thought into that. I have no idea to be honest. I always assumed that it was a card bought at a store and then some of the things were pasted on. Does anyone else know the answer to the question?

That's always been my understanding as well. Having said that, it seems odd to me that nobody's ever been able to track down a copy of the original card...or, at the very minimum, determine which company was responsible for its production.

"The Halloween card was a standard Halloween card of the time, with a few 'additions' created by Zodiac to scare Avery. Zodiac wrote "you are doomed" (in the hole within the tree). He also included an odd symbol, and on the back of the card, a message that mentioned the various ways he could presumably kill people, By Knife, By Rope, By Fire, By Gun."

zodiac added the pumpkin over the groin and the hanging skeleton on the inside to the card, he also added the 12 peeping eyes.

^ Thats the most popular theory. I have also been trying to find the original unmarked card for a few years now but no joy as of yet.
